Download Application Materials

You can download an AIM application, recommendation forms, medical forms, budget, checklist, and other important papers for AIM by clicking on the links below. In order to download you must right-click on the link and select the "Save Target As..." or "Save Link As..." option. You will then be asked where you want to save the file. Make sure to remember where you are saving it to so that you can find it later. If you don't already have Adobe Reader software, you can download it for free in order to read and print these files. We have provided a link that will take you to the Adobe Reader website.
